About the Role
We’re seeking a Nurse Practitioner to join a collaborative primary care team in a full-time outpatient setting. This role offers the opportunity to provide comprehensive care for patients of all ages, with a focus on quality, continuity, and patient-centered treatment.
What You’ll Do
- Perform patient histories, physical exams, and preventive screenings
- Diagnose and manage acute and chronic conditions across diverse populations
- Order, interpret, and follow up on diagnostic tests
- Collaborate closely with physicians, nurses, and specialists to coordinate care
- Document all patient interactions accurately in the EMR
- Participate in quality improvement initiatives and team-based care planning
- Help connect patients to community health and wellness resources
What You’ll Need
- Active and unrestricted Nurse Practitioner license
- Current national board certification and DEA registration
- CPR certification
- Experience in outpatient or community health settings preferred
- Strong clinical judgment and the ability to work independently and as part of a team
