Nurse Practitioner / Physician Assistant – Geriatric Care Focus
Support older adults through proactive, team-based care that prioritizes dignity, independence, and quality of life. We’re hiring a Nurse Practitioner or Physician Assistant to join a collaborative care team serving aging adults with complex medical needs.
This role centers on building relationships, coordinating care across disciplines, and managing chronic conditions in a way that keeps patients healthier and at home.
About the Role
- Deliver primary and preventive care to older adults within a structured care model
- Conduct assessments, exams, and routine medical procedures
- Order and interpret lab work, manage medications, and respond to urgent needs
- Collaborate with physicians, nurses, and social workers to develop care plans
- Provide education and support to participants and families
- Participate in care planning, quality improvement efforts, and after-hours coordination as needed
What You'll Need
- Active certification and licensure as a Nurse Practitioner or Physician Assistant
- Experience working with geriatric or medically complex populations preferred
- CPR and First Aid certification
- Ability to work effectively in a team setting and build rapport with patients
- Comfortable managing care across home, clinic, or community-based settings
- Valid driver’s license and ability to travel locally when needed
